CATZ Athlete Information Form
Each new athlete is required to complete one of these forms before they begin their CATZ training. If the athlete is under the age of 18 a parent or guardian must sign the release page for them. Please include emergency contact information. E-mail addresses are advised as we usually send out facility hours, closings and services information using this medium...we do not share it with any other company.
